Breakthrough pain (BTP) — ขนาดยา prn
หลักการปรับยา: ลด dose 25-50% เมื่อเปลี่ยนระหว่าง opioid ต่างชนิด (incomplete cross-tolerance) • ในผู้ป่วยเปราะบาง / สูงอายุ / ตับไตบกพร่อง ลด 50% • ESRD (GFR < 15) ลดเหลือ 25% สำหรับ morphine • ขนาดที่น้อยสุดของ Mo ไม่ควรต่ำกว่า 4-8 mg/day

Tramadol IV : Tramadol PO = 1 : 1.5
Tramadol PO : Morphine PO = 5 : 1
Codeine PO : Morphine PO = 10 : 1
Morphine PO : Morphine IV/SC = 3 : 1
Morphine PO : Fentanyl IV/SC/transdermal = 100 : 1 (mg→mcg)
Morphine PO 60 mg/day = Fentanyl patch 25 µg/hr
